Added Value Accounts - apply by post

1) Download the appropriate application form for Gold or Platinum in pdf format by clicking on the appropriate 'download' link below*.

Click here to download the Platinum account application form.

2) Please print off the relevant Added Value Account important information, or save it to your hard disk, before applying.

3) Print the application form on plain white A4 paper using black ink.

4) Complete the application using black or blue ink pen so that the information is legible.

5) Sign and date the application form.

6) Post the application form to the following address:
